Job Title: Home Health Wound Care Registered Nurse (WOCN)

Location: Philadelphia, Pennsylvania; occasional travel to surrounding counties may be required

Requirements

  • Education: Graduate of an accredited school of nursing; BSN preferred
  • Licensure: Current and valid Registered Nurse (RN) license in the state of Pennsylvania
  • Certification: Certified Wound Ostomy Continence Nurse (CWOCN) certification required
  • Experience: Minimum of two years of experience as a Registered Nurse in a relevant clinical setting; prior experience in wound care, ostomy care, and/or continence management preferred
  • Skills: Excellent assessment, critical thinking, and problem-solving skills; strong interpersonal and communication skills; proficiency in electronic medical records systems; ability to work independently and as part of a team; current CPR/BLS certification; valid driver's license and reliable transportation

Position Details

  • Responsibilities:
    • Conduct comprehensive assessments of patients' wounds, ostomies, and continence issues
    • Develop individualized plans of care in collaboration with physicians and interdisciplinary teams
    • Select appropriate wound dressings, therapies, and equipment based on evidence-based practice guidelines
    • Educate patients and families on wound care techniques, ostomy care, and continence management strategies
    • Provide direct patient care, including wound dressing changes, ostomy care, catheter care, and skin care
    • Monitor patient progress and adjust care plans as needed
    • Provide emotional and psychosocial support to patients and families
    • Participate in quality improvement activities related to wound care, ostomy care, and continence management
    • Maintain accurate and timely documentation of all patient assessments, interventions, and outcomes
